Freedom of Information Requests

Any person requesting records of the Homewood-Flossmoor Park District may make such a request in writing either in person, by mail, email or facsimile to the Freedom of Information Officer, Doug Boehm, at the main office located at 3301 Flossmoor Road, Flossmoor, Illinois, 708.957.8574 (fax) or email foia@hfparks.com. (See Exhibit A form, Request for Public Records for a sample request.) If the Freedom of Information Officer is not present in person, the Superintendent of Administration or the Executive Secretary will handle the request. If records must be certified, this must be indicated in the request. The fees for any such records, if the person requesting the records wished them to be copied, are as follows:

• No charge for the first fifty (50) black and white pages
• Over fifty (50) black and white copies: 1 cent per page
• Color copies: 6 cents per page
• Electronic copies: cost of the medium
• $1.00 to certify copies
